Lack of images to support documentation and evidence progression / deterioration of
ulceration.
Onwards referrals to TVN and safeguarding not completed in a timely manner which
may have impacted on care plan / progress.
Risk assessments not completed in a timely manner - staff to be made aware of the
correct timescale and risk assessments to complete.
Lack of photographic images/information within documentation to evidence risk
assessment, monitoring, progress/deterioration, referrals and escalation to
necessary MDT and services.
Escalation and reporting of new pressure ulceration not completed in a timely
manner.
An important way of improving our services is receiving and acting on feedback from our
service users, patients and carers. The Friends and Family Test (FFT) is available online for
Trust and ECP services as well on paper and in other languages.
Trust-wide Patient Safety Learning Event: Making Medicines Safer - topics include Human
Factors approach to understanding medicines safety and reducing errors, local medicines
safety projects using QI approaches. Tuesday 29th March 1:30-5:00pm via MS Teams. Link
