I have had such excellent support, instruction aid and help in (making

LTR Two (Local Team Review) - 51-year-old man who was
under District Nursing (DN) care with a healing category 4 pressure ulcer on his
sacrum and lower back and a healing category 3 pressure ulcer on his hip - presented
with unstageable pressure ulcer on his upper sacrum.
Patient factors in development/deterioration of the pressure ulceration: past medical
history - multiple sclerosis, contracture of limbs, smoker, infrequent position changes
(had a lateral turning platform to offload pressure) stayed on his back at all times,
raised Walsall score, existing pressure damage, reduced oral intake, low weight, faecal
incontinence, has supra pubic catheter in-situ which had bypassed at time causing
moisture and MASD risk, scoliosis of spine. Patient has full package of care, was
nursed on electric community bed, alternating mattress, lateral turning platform, satin
sliding sheets and Prevalon foot protectors. Last visit patient had a chest infection
and was escalated to hospital and safeguarding completed was they were initially
refusing to go.
