Patient Safety Newsletter 2022
Learning from Incidents and Feedback
REFLECT, LEARN, GROW.
Volume 1 - Issue 3
Newsletter date: March 2022
Congratulations to the Southall District Nursing Team on winning Team of the Month February
As you are all aware we strive to learn lessons from incidents/feedback as we recognise this is a part of
keeping our patients and staff safe. Unfortunately, despite our best efforts, things can happen. Within this
newsletter, you will see examples of lessons learned.
I would like to thank you for all the hard work you do to keep our patients safe Dr Claire
Dillon, Clinical Director of Integrated Care Services.
As a service we strive to learn lessons from incidents and events as we recognise that this is an
important part of keeping our patients and staff safe. We look after a wide range of services but I
hope that it will be helpful to see learning across the whole of Integrated Care as there are often
common themes.
Despite our best efforts serious incidents do occur, or complaints are received, and it is our
responsibility to ensure we learn from these and that we are open and transparent about this
learning. Over the next few pages you will see the first edition of the newsletter and some recent
learning. It would be great to get your feedback and I am also keen to include pockets of good
practice and things that you would like to celebrate about your service - so please do share these
with me.
Despite the recent challenges it is lovely to see the Southall District Nursing Team win the Team
of the month. Well done to all involved.
Dr Claire Dillon - Director of Integrated Community Health Services
